I ask all clients I work with to start with and sign this intake. I will have copies for you at the start of each session, or you may print it out yourself and bring it in with you.
Optionally, for a more in-depth opportunity to check in about your session preferences, I´d love for you to check out this Preparing for Bodywork Mini-Workbook. This may beespecially helpful if you are newer to the experience of massage, and unsure of what to expect or of what your preferences might be.
Know that we will at least briefly discuss its contents each session, whether you choose or are able to consult it beforehand :) But many have found it useful to consider these questions in their own space and time, outside of the session itself!
You are also more than encouraged to bring a filled printout of this workbook to me for your session, or bring just your written or verbal answers, or to fill out the copies I'll have with me at the start of your session.
Notes on Consent-Centered Praxis
Especially as a survivor of sexual and physical violence, I know first-hand both the incredible healing power, and potentially heavy impact, of touch & bodywork. As a practitioner, I must be constantly learning, healing, and adapting to best facilitate the work that I know can re-empower us to expect and demand feeling seen, listened to, cared for, & encouraged to know what we want, in our bodies.
I want my, and all, clients to know that they have the right to hold me and their practitioners accountable for this at all times. Your sessions are meant to be designed and sovereigned by YOU, not the practitioner. We are not the experts on YOUR body. We are facilitators and supporters of your own autonomy and growth, which NEVER needs to include you feeling unsafe, or pressured into something without your consent.
I love, invite, and thank, everyone for the opportunity to hear your feedback and questions about my services, my website, and these forms. I may never be a perfect listener, but I take the practice of it seriously! Contact me directly, or please check out thisanonymousBodywork Services Survey, if you'd like to gift me your voice so I might continue to improve on meeting your needs. Thank you!
